About this role

At the Care and Social Services Authority in Vellinge Municipality, a team consisting of a senior case manager, eight social services case managers, three LSS case managers, a fee manager/housing coordinator, a fee manager/elderly and family navigator, and two senior contacts is currently working. Vellinge Municipality's Care and Social Services Authority is now seeking three committed and flexible social services case managers for temporary positions lasting 6 months up to one year. JOB RESPONSIBILITIES Our target group includes municipal residents who need support and assistance based on current legislation such as the Social Services Act and the Transportation Services Act. The temporary positions involve case management work across multiple areas within the Care and Social Services Authority. This includes interventions under the Social Services Act (SoL), Transportation Services Act, and the SVU process, which ensures a smooth and secure transition for patients from inpatient healthcare to home or municipal care. Your task is to investigate, make decisions, follow up on interventions, and be responsible for information and collaboration during ongoing investigations. This may involve transportation services, home care, day activities, short-term accommodation, alternating residence, specialized housing (dementia or somatic care), or short-term care. The work is based on a holistic perspective centered on the individual's needs, often involving collaboration with relatives, healthcare providers, health services, other authorities, and partners. The social services case manager role involves meeting diverse individuals, requiring strong social skills, attentiveness, and an interest in creating positive interactions. You enjoy meeting people, have a positive outlook, and possess good collaborative abilities with personal drive. Success requires the ability to create clarity and trust. In interactions or dialogues with individuals and their families, a dignified approach and warm interest in working with people are crucial. The work demands independence, flexibility, accountability, and excellent communication skills, both verbally and in writing. Beyond formal qualifications, significant emphasis will be placed on personal suitability. QUALIFICATIONS Education: We seek candidates with a social work degree, social care program diploma, or equivalent education as determined by the employer. Knowledge: Extensive understanding of current legislation and legal practice in the field, with experience in administrative work under SoL and the SVU process. Strong communication skills, both verbal and written. Experience with IBIC is desirable. Good IT skills. Experience with Lifecare and Mina Planer systems is advantageous.
