Within the health and medical care administration, primary care, dental care, and hospital care are combined into a single administration and organized into regional and local healthcare areas. The Skellefteå local healthcare area includes the Emergency Department, the Center for Surgical Operations, the Medical and Geriatric Clinic, the Rehabilitation Center, the Psychiatric Clinic, 1177, as well as primary care in Skellefteå and Norsjö. We are now seeking a Care Planning Coordinator who will be part of a context where you will participate in the development of our own working methods and processes. You will join an organization that collaborates within and between each other along with the municipalities of Skellefteå and Norsjö, thus providing the best possible conditions to deliver the care requested by our citizens. JOB DESCRIPTION
As a Care Planning Coordinator with us, you will contribute knowledge and engagement that makes people feel secure.
As a Care Planning Coordinator in the Skellefteå Local Healthcare Area, you will have overall control and coordination regarding the discharge process. You will ensure a safe discharge process with secure homecomings and significant patient involvement. The discharge should provide the receiving care provider with good conditions to deliver safe and secure care and support. You are responsible for staying updated on laws and agreements that regulate collaboration between healthcare institutions and to a large extent contribute to our compliance with our part of the agreement. As a Care Planning Coordinator, you will train new staff, across various categories, in the Lifecare SP tool and in the discharge process. Through your expertise, you will support and guide patient-responsible staff in how planning is carried out and can contribute where planning is complicated, acting as the hub between the hospital, health centers, and municipalities.
In your work, you will use Lifecare SP and Cosmic and have contacts with relevant parties. Through your efforts, you will free up time for the area’s healthcare staff with the aim of increasing patient-facing time and ensuring that no one remains in the hospital unnecessarily.
You will also work closely with the hospital coordinator, and you will substitute for each other as needed or in case of absence.
QUALIFICATIONS
You are a licensed nurse or have other relevant experience in healthcare. Experience in care planning, teamwork, or pedagogy is an advantage.
In addition to your medical expertise, we expect you to have good collaboration skills and the ability to work independently. You can listen and communicate with both patients, relatives, and colleagues in the workplace. As a person, you are engaged and positive. Your ability to network is important, and we welcome your interest in contributing your knowledge to develop the operations in this area.
