About this role

Do you want to make a difference in patients' daily lives and work in a small, dedicated team? Then this could be the position for you! **Job Responsibilities** As a nursing assistant in the Home Care Team in Sala, you will have a varied and independent role focused on patient-centered care at home. Your responsibilities will include answering incoming phone calls, providing immediate information, or referring patients to a doctor or nurse as needed. You will conduct home visits where you perform tasks such as sample collection and wound dressings. Some home visits will be carried out in collaboration with other team members. An important part of the job is also to monitor waiting lists and schedule appointments for patients with doctors and nurses in the Cosmic scheduling system, as well as to follow up on discharges from inpatient care via Link. You will be involved in coordinating and planning patient care alongside nurses and doctors in the team. The role will also include assignments from specialist care and advanced home healthcare (sample collection, injections, wound dressings, etc.). The work requires close collaboration with various professional groups, which demands both independence and strong teamwork skills. **About the Workplace** The Home Care Team in Sala was established in October 2021 as the first of the region's four teams and has demonstrated positive results, including a reduced need for inpatient care. Patients have also reported feeling increased security since joining the Home Care Team. The team consists of doctors and nurses from the region, working closely with the municipal home healthcare services. A social worker is also part of the team. Home rehabilitation is responsible for rehabilitation efforts in patients' homes. We also collaborate with healthcare centers in Sala and other units within our clinic (Primary Care East). Patients are received by referral. After an assessment visit, the team takes on the coordinating and overall medical responsibility at the primary care level. The Home Care Team also accepts assignments from other clinics, such as infusions and injections. With us, you will work in a small, engaged team where the focus is on providing solutions at home for patients with more extensive care needs. We have short lead times and close collaboration between doctors, nurses, and municipal home healthcare, which allows for quick decisions and reduces the need for inpatient care. You have a good ability to communicate in Swedish, both verbally and in writing, which is essential for safe and secure patient care. Experience in home healthcare or caring for severely ill patients is an advantage, as is previous experience with the Cosmic medical record system. As a person, you are proactive, self-driven, and have a knack for collaborating with others. You build relationships easily and thrive in a role where you meet patients in various life situations.
