We Are Seeking an Engaged and Professional Social Worker for Our Advanced Home Healthcare Team (ASIH) About Us Aleris Närsjukvård AB operates Advanced Home Healthcare (ASIH), inpatient palliative care, Rehab Station, urgent care clinics, and basic home healthcare. A clear healthcare concept with good collaboration enables safe patient transitions. Our employees are driven by high engagement and involvement. We make a difference every day. Within Aleris Palliative Care North, we care for critically ill patients in their homes (ASIH). Care is provided through healthcare choice on behalf of Stockholm Region. We offer specialized care tailored to each patient's needs, with the goal of providing qualified medical and psychosocial care around the clock. Our service cares for adult patients with various diagnoses, most commonly cancer, COPD, heart failure, and neurological diseases. We have a holistic view of the individual, focusing on creating round-the-clock security for patients and their families through high accessibility, participation, and continuity. Working in palliative care and ASIH means working preventively, providing individualized care, and helping create a sense of security. The Role With us, you'll have a job characterized by teamwork, patient-centered care, and opportunities for professional development. As a social worker, you will provide qualified social support to help patients and families manage and process their emotional, social, and practical situations arising from illness, injury, and life changes. You'll offer expert guidance on issues such as illness-related crises, family conflicts, and social challenges. There is a strong emphasis on interventions that create meaning and enhance quality of life. We work in patients' homes but also offer on-site and digital consultations. You'll be part of our paramedical unit with colleagues in various professions including physiotherapists, occupational therapists, social workers, and dietitians. You'll have two social worker colleagues within Aleris ASIH North for support. Beyond the paramedical group, you'll collaborate closely with nurses, doctors, and nursing assistants, participating in weekly team rounds. Personal Qualities We seek someone with a flexible approach, who is attentive and proactive. We want someone comfortable in their professional role, who collaborates easily and enjoys teamwork. You should be able to work both independently and in a multi-professional team.
