About this role

Are you a structured and driven social worker with knowledge in the field of disabilities? Then you might be the person we are looking for to fill our position as a coordinator for escort and relief services. As a coordinator, you will be a key player in creating quality of life for our citizens. You will be responsible for implementing measures that break isolation, enable activities, and provide relief at home. You will work in a team consisting of coordinators and a unit manager. You will work independently but have regular check-ins with your colleagues each week. Every other week, the unit manager will also participate in these meetings. Within the larger organization, the Support and Service Unit is located at three workplaces in Östersund, where several professionals, including social workers, psychologists, speech therapists, special educators, and counselors, provide operational support to staff working within the Disability Sector. You will work from an office at Krondikesvägen 60, which you will share with your closest coordinator colleagues. The work is scheduled during daytime hours on weekdays, and you will have an annual working time model that includes 40 hours per week, allowing for good opportunities for flexible working hours. Evening work may occur on rare occasions throughout the year. Your new job In this role, you will implement escort and relief services according to the Act on Support and Service for Certain Disabled Persons (LSS) and the Social Services Act (SoL). A significant part of the work involves recruiting, interviewing, and matching staff to approved services. Therefore, knowledge and/or experience in the field of disabilities is valuable for a good understanding of, for example, diagnoses and the needs that follow these diagnoses. It is also beneficial if you have prior experience in social work with families or youth. Since the team is small, it is important that you possess good collaboration skills. Travel for home visits to citizens is part of the job, and a B driver's license is an advantage. You will be responsible for documentation and follow-up during ongoing services. In your work, you will often collaborate with assistance officers or LSS caseworkers, and to succeed in this role, it is important that you have the willingness and ability to cooperate with other professionals as well as experience in conducting meetings. You will also participate in meetings aimed at establishing a coordinated individual plan for citizens, known as SIP meetings. The role also includes some personnel responsibility, as you are expected to be available for those working as escorts or relief workers regarding questions about, for example, salary or sick leave, of course in consultation with and with close support from the unit manager. You will also be responsible for training and continuously guiding the employed escorts and relief workers in their work, often in connection with workplace meetings. Therefore, we are looking for a person with drive and commitment who also has a good ability to collaborate with other professionals. You will belong to the Support and Service Unit, which is part of the Disability Sector within the Health and Care Administration. Common tasks • Administer incoming expressions of interest for escort and relief services in the recruitment system ReachMee • Interview and reference checking of applicants for escort and relief assignments • Matching new and existing escorts and relief workers with citizens' approved services • Guidance and training of escorts and relief workers • Social documentation in Viva • Collaboration with LSS caseworkers and assistance officers The unit manager may delegate other tasks in addition to this.
