**Description**
Backa Healthcare Center is a newly established healthcare facility located in the Backa area, approximately 2 km east of Karlskoga Center.
Our goal is to grow into a leading healthcare center that offers excellent primary care for everyone and a workplace where employees thrive.
We are currently seeking a social worker/counselor who wants to join us on our journey to develop the future of primary care. You share our belief that approach, quality, and continuity place the patient's needs at the center. Together, we have built a highly skilled team consisting of specialists in general medicine, nurses, district nurses, diabetes nurses, nursing assistants, medical care administrators, therapists, and physiotherapists. At Backa Healthcare Center, we also provide child and maternal health care. We work interprofessionally, where everyone has significant opportunities to influence the work environment and participate in shaping our methods of operation.

**Responsibilities**
The responsibilities primarily include assessment, counseling, investigation, treatment, and rehabilitation both individually and in groups. We also hope that you will serve as the rehabilitation coordinator for the healthcare center, as part of the rehabilitation process and support during sick leave to facilitate the patient's return to work.
As a social worker/counselor at the healthcare center, you will be part of a large network around the patient, both within the organization and with other stakeholders and authorities. You will play a central role in collaboration for the patient.
**Qualifications**
You are a trained social worker, preferably a licensed healthcare counselor with experience in primary care. You have completed level 1 training in CBT. Experience as a rehabilitation coordinator is an advantage.
**Personal Attributes**
You are self-motivated, responsible, and flexible with a good approach towards both patients and colleagues. You enjoy working both in teams and individually and are eager to be involved in development work. You have strong skills in Swedish, both spoken and written, and additional language skills are a plus.
**Conditions**
Working hours: Daytime. Full-time.
Employment type: Permanent with an initial probationary period.
