A transformation to Proximity Healthcare is underway in Sweden.
Proximity Healthcare is a health and medical care system where regions and municipalities work together for our patients' best interests. At Fagersta Municipality, the municipal healthcare collaborates with Region Västmanland, Norberg municipality, and Skinnskatteberg municipality. This means that healthcare and social care in Fagersta municipality are now changing and developing their mission. The municipal healthcare is responsible for nursing homes, short-term units, safe discharge, home healthcare, LSS, and psychiatric care. The municipality's nurses also closely collaborate with the Fagersta Proximity Care Team. The team's purpose is to offer residents the right care, at the right time, and at the right care level. It focuses on our most seriously ill elderly and AH patients, regardless of their living situation. The team is staffed by nursing assistants, nurses, and physicians from inpatient care. The Proximity Care Team works during daytime, Monday to Friday, but team nurses also work evenings.
Your Mission
Your focus is safe care for the patient's best interests. Working as a nurse in the municipality is varied and very flexible. You will distribute work yourself based on emerging patient needs. Your work will be diverse, educational, and challenging. You will make good use of your experiences and competencies, and you will also acquire new knowledge with us.
You and your colleagues will help implement daily work and collaborate with experienced nursing assistants, physiotherapists, and occupational therapists. You will coordinate, lead, and be responsible for planning, implementing, and evaluating healthcare interventions and nursing measures in our area. You will collaborate with the municipality's coordinator, regional care coordinators, discharge coordinators, and Fagersta Proximity Care Team.
Work is distributed across day, evening, and weekend shifts.
The municipality has signed a local collective agreement for Academic Specialist Service (AST) with the Nursing Association to enable further education during work hours.

Your Qualifications and Merits
Registered Nurse
Class B driver's license required for the position
Previous experience in municipal operations is meritorious.
