Psychiatry in Region Norrbotten has begun a transformation towards Near Care, where we are moving healthcare into the home close to the patient, and here at the General Psychiatry Clinic in Piteå, there is a strong sense of community and a constant desire for improvement. General Psychiatry Piteå is a specialist psychiatric outpatient clinic consisting of multiple competencies that perform assessments, treatment, and follow-ups for people over 18 with moderate to severe psychiatric difficulties. We receive patients at our clinic and also provide outreach services. For this mission, we have established collaborative networks between ourselves, other operations within Region Norrbotten, such as other psychiatric areas of operation, Piteå-Älvdalen health centers, Piteå Municipality, and other healthcare services. As a clinic with approximately 70 employees, we must work together to best meet our mission. To lead our 2-year development project, where the clinic will examine how we can provide care close to the patient and create enhanced transitions between care providers, we are seeking you who are a licensed nurse or trained social worker and who are passionate about development and quality care. This is particularly relevant for transitions from child and adolescent psychiatry to us and between us and primary care. The project will be conducted jointly with Child and Adolescent Psychiatry (BUP), where project employees from both units will create a joint project plan for implementation, which will involve developing collaboration, structure, and routines to find the best working methods. We are seeking: - A licensed nurse or trained social worker - Advanced qualifications in psychiatry, psychiatric specialist nurse, or licensed healthcare counselor are advantageous - Excellent Swedish and psychiatric terminology skills are required - Knowledge of minority languages and journal systems VAS or Cosmic is meriting, as is experience in psychiatry, healthcare, coordination, and/or development leadership We place great importance on personal qualities. You are flexible, persistent, and quality-conscious. You have good collaboration and problem-solving skills, can see the big picture, and consider broader perspectives. What you will work on: The clinic has identified a need to develop a secure and comprehensive care transition for patients transferred from child and adolescent psychiatry to adult outpatient psychiatry. We also see a need for enhanced care transition between adult psychiatry and primary care. Adult Psychiatry and Child and Adolescent Psychiatry will jointly conduct a 24-month project where responsible project planners from each unit will develop a project plan, create collaboration structures, and establish routines for optimal working methods.
