Every day. Are you a nurse looking to work with specialist care at home and help develop our unit? Then you should apply to us and our expanding local healthcare team, which is being enhanced to increase opportunities for patients in need of inpatient care to receive specialized care at home.
As the local healthcare team transitions from the geriatrics area to the acute care area, the team is also being expanded to offer increased home care options for certain patients. At the same time, the local healthcare team already plays an important role for patients at the threshold between inpatient and outpatient care.
The inspiration for this expanded team originally comes from other countries where the "Hospital at Home" concept is successfully implemented. Studies show several positive effects for patients receiving care in their own homes, such as improved eating and sleeping, increased activity, and a reduced risk of acute confusion or falls. Experiences so far indicate very high patient satisfaction and involvement.
Job Responsibilities
The team consists of nurses and doctors. We are now looking for you, a nurse, who wants to be part of our local healthcare team. Instead of the patient being admitted to the hospital, we, as healthcare professionals, go to the patient's home to conduct assessments and provide treatments. This may involve patients who can completely avoid hospital care or patients who can be discharged earlier from inpatient care. The position primarily involves working with patients in their homes. The patients reside within our catchment area, and you will travel by car.
New working methods and the development of the unit will gradually be shaped together, which requires you to be curious, enjoy development, and take significant responsibility. You need to be a team player with good collaborative skills while also being confident in your professional role and able to work independently. As the local healthcare team moves to the acute care area, we will work closely with the emergency department, and we welcome a combined position between these two units.
Qualifications
- Licensed nurse with professional experience.
- A driver's license is required.
- Specialist training in infection, medicine, elderly care, or similar is advantageous. Experience in acute care, somatic inpatient care, and/or municipal health and medical care is also beneficial.
- You are someone who sees opportunities instead of obstacles.
- You are driven by new challenges, can quickly adapt, and have an open mind to learn new things.
