ÄlSa is a geriatric home visit team for elderly patients with multiple chronic conditions. The team offers connection to ÄlSa for a limited period for patients with complex care needs and repeated hospital admissions. The home visit team has a clear goal – to create security and make daily life as good as possible for those enrolled and their relatives. The team consists of nurses and geriatric doctors who work closely and effectively together. We operate during daytime hours, Monday to Friday, as well as on weekdays without holidays.
We are now seeking a nurse or district nurse to join our team! The position is a permanent full-time employment with a temporary placement at ÄlSa until December 31, 2026.
Nurse/District Nurse for ÄlSa Team, Örnsköldsvik
Job Responsibilities
As a nurse at ÄlSa, you will conduct home visits either independently or together with a doctor. A significant part of the role involves providing support to patients and their relatives through phone contact or home visits. Your role will include being a cohesive link between the hospital/health center and home healthcare. You will work with:
• Monitoring vital parameters and taking samples at home
• Assessing symptoms and implementing actions
• Administering medications at home
• Following up on completed actions
• Assessments according to Senior Alert (nutrition, pressure ulcers, fall risk)
• Conducting medication reviews together with doctors
The introduction will be individually tailored and will take place over two weeks.

Qualifications
We are looking for someone who:
• Is a licensed nurse
• Has good knowledge of Swedish, both spoken and written
• Holds a valid driver's license (B category)
It is advantageous if you:
• Are a trained district nurse or specialist nurse with a focus on elderly care
• Have work experience in elderly care
