The specialist care is responsible for specialized healthcare in Västernorrland County and is organized into county operations based on various medical specialties. Our hospitals are located in Sundsvall, Sollefteå, and Örnsköldsvik.
The County Psychiatric Services offer care at five locations: Sundsvall, Härnösand, Örnsköldsvik, Kramfors, and Sollefteå. We have four psychiatric care departments and ten outpatient clinics in the county. Our target group includes individuals aged 18 and older with psychiatric disorders or mental health issues that require specialist competence.
Specialized psychiatry works with prevention, diagnosis, investigation, care, and treatment of mental illness and psychiatric disorders. Psychiatric emergency services are available in the evenings, nights, and weekends, and all involuntary care is located in Sundsvall, covering the entire county.
We are now seeking a Coordination Coordinator for the Psychiatric Outpatient Clinic A in Sundsvall. This position is a permanent full-time role with a start date in January 2026 or as agreed.

Job Responsibilities
• Coordinate collaboration between internal and external stakeholders during discharge from inpatient care.
• Ensure that the clinic's mandate as a primary care contact is followed according to the "Act on Collaboration during Discharge from Inpatient Healthcare and Transition to Open Forms of Involuntary Care."
• Provide support to colleagues in both inpatient and outpatient care regarding collaboration issues.
• Contribute to collaboration as part of the dependency plan.
• Participate in the development and adaptation of the mandate based on the needs of the operation in dialogue with the operations manager and unit manager.
We offer flexible working hours, Monday to Friday from 8 AM to 5 PM, with the possibility for individual adjustments.

Qualifications
We are looking for someone who:
• Holds a degree in social work or a similar accredited professional qualification.
It is advantageous if you:
• Have education in rehabilitation science and addiction/dependency.
• Have experience in social services and psychiatry.
• Have previously worked with addiction/dependency.
