About the Open Position
In 2020, legislation was introduced requiring regions to offer patient coordination services.
These services are offered to patients with a specific need for individual support to return to, remain in, or enter the workforce/studies. As a rehabilitation coordinator, you play a central role in supporting patients on sick leave in their path back to work or studies. You coordinate healthcare interventions, collaborate with external actors like the Social Insurance Agency, Employment Agency, and employers, and provide individual support to patients throughout the rehabilitation process.
The position will also include some social work, particularly during periods with less need for coordination services, and to some extent continuously to maintain competence. Social work includes informative and practical support as well as psychosocial treatment work.
About Us
Our social work clinic employs 13 staff, including one rehabilitation coordinator and the remaining social workers. The rehabilitation coordinator and social workers collaborate closely, using joint referral reviews for case allocation and shared reflection on complex cases.

Your Profile
We seek someone with a social work degree and experience involving collaboration with internal and external actors. Deeper knowledge in rehabilitation and/or insurance medicine is desirable, and previous experience with individual support is preferred. Additional qualifications include courses/training in rehabilitation coordination in healthcare regarding sick leave and rehabilitation processes. You should have documentation experience and participation in development work. You're comfortable with digital workflows in patient work and organizational routines and improvement efforts.
As a person, you can see the big picture while working independently with clear structure and focus. You can prioritize, set boundaries, and make professional assessments based on patient needs. You're personally secure, good at setting boundaries, and maintain focus within the assignment's framework. Personal suitability is crucial.
