Rehabilitation Coordinator at Kumla Health Center
Job Responsibilities
We are now expanding our operations with an additional rehabilitation coordinator. The purpose of a rehabilitation coordinator within healthcare is to provide coordination efforts for sick-listed patients to promote their return to or entry into the workforce. Coordination efforts should be provided as needed within healthcare and consist of personal support, internal coordination, and collaboration with other stakeholders. Appropriate, timely, and secure sick leave means that it is a conscious part of treatment. It is issued based on the right indications and according to local and national guidelines, including the National Board of Health and Welfare's decision support. Each sick leave is individually tailored and accompanied by a plan for returning to work. As a Rehabilitation Coordinator at the health center, you will work independently but also in a team with doctors, physiotherapists, and our psychosocial team. As a rehabilitation coordinator, you will:
• Support the health center's work in developing the sick leave process by creating, implementing, and following up on routines for this.
• Early on, identify and engage patients who need coordinated efforts and support these patients during their sick leave.
• Map patients' rehabilitation needs and contribute to the establishment of a rehabilitation plan in dialogue with the patient, the doctor, and other relevant healthcare contacts or stakeholders.
• Be a partner and contact person for external stakeholders such as employers, occupational health services, the Social Insurance Agency, the Public Employment Service, and social services.

Qualifications
We are looking for someone who is, for example, a physiotherapist, nurse, occupational therapist, social worker, or has another academic qualification that the employer deems equivalent, and has experience in relevant areas within the profession. Experience in rehabilitation coordination is an advantage. To succeed in the role of rehabilitation coordinator, you need to be proactive, responsible, independent, driven, and have a good ability to collaborate with other stakeholders both within and outside the organization. You should have a good understanding of the various support systems in society and an interest in issues related to insurance medicine, occupational health, gender equality, domestic violence, and labor law. We will place great emphasis on personal suitability.
