We work on behalf of the Social Psychiatry Authority to provide high-quality housing support and coordination to the municipality's residents. Our target group is 18-65 years old with serious mental health issues and complex social needs, where interventions are decided according to the Social Services Act. The work is performed in clients' own homes and in our centrally located facilities. Through the rehabilitation process, we continuously aim to strengthen what works and support clients towards further independence. Your Responsibilities As a rehabilitation assistant at SPR, you will be part of one of two teams at the unit. You will be responsible for a number of your own clients, with planning and implementation based on the individual assignment. This also includes documentation such as social journals, implementation plans, goal follow-ups, etc. You will have obvious support from colleagues, team leaders, and managers. The work is flexible and varies from day to day. Based on the client's unique needs, this can involve anything from household management to supporting healthcare and authority contacts or coordinating larger meetings with external participants. Client contact is central, but an important part of the work is also collaboration with entities like Psychiatric Center Södertälje and forensic psychiatry. Who Are You? Personal Characteristics: - You are flexible and responsible, independent in your work - You are a secure person who can handle meeting people in crisis - You are solution-oriented and have a good approach towards colleagues and clients - You are unpretentious and see all people's equal value as a natural part of your value base Requirements: - You are a social pedagogue or have an equivalent education - You have documented experience working with the target group - Since we work in a large geographical area, a B driver's license is required Meritorious: - Training and experience in low-affective approach - Training and experience working with harmful use and dependency - Training in MI, ESL, and IBIC We place great importance on your personal characteristics. The unit has 21 employees: 17 rehabilitation assistants, one coordinator, two team leaders, and one unit manager.
