Close Care is a health and medical care model that uses primary care as its foundation, where the region and municipalities interact clearly based on individual needs. This means that healthcare and support in Fagersta municipality are now developing their assignment within the healthcare group. The municipality's rehabilitation team works across various areas including specialized housing, short-term units, safe discharges, home healthcare, LSS, psychiatry, and home visits in regular housing.
Your Mission

The work involves creating a meaningful everyday life for our Fagersta residents based on individual needs, by working preventively and long-term. The role involves working in teams with various professional disciplines in close cooperation with care staff and colleagues in the healthcare group.
You will have a close collaboration with your colleagues in the rehabilitation team, which consists of physiotherapists, occupational therapists, and rehabilitation assistants. The role also requires flexibility and collaboration with colleagues in the workgroup to cover for each other as needed.
Your assignment also includes participating in the planning of safe discharges, collaborating with the municipality's coordinator, the region's care coordinator, and the discharge coordinator.
The work as a physiotherapist within the municipality is varied, challenging, and allows a high degree of independence, as you allocate tasks according to arising needs.
Tasks included in the assignment are, among others:
- Assessing needs and determining appropriate treatments/compensatory measures based on a function-preserving approach.
- Trialing, prescribing, and following up on aids.
- Training care staff in transfer techniques and other profession-specific areas.
You will guide, educate, and delegate tasks continuously to rehabilitation representatives and care staff to provide quality care to our Fagersta residents. Work hours are during the day. Upon starting your employment, we have prepared an introductory program for you.
Your Qualifications and Credentials
The position requires you to be a licensed physiotherapist and to hold a Class B driving license.
