Then this could be the job for you! The Nephrology Department in Lund is responsible for specialized nephrology care and advanced nephrology services in the southern healthcare region. The department includes two hemodialysis clinics, an inpatient ward, and a nephrology outpatient clinic. Additionally, there is a clinic for training and follow-up of patients on peritoneal dialysis and home dialysis. Kidney and pancreas transplants for our patients are performed in Malmö, while follow-up care takes place in Lund. We are a clinic with a positive atmosphere and a strong focus on close collaboration among professionals. Our outpatient services see patients for regular visits, consultations, treatments, or rehabilitation. Here, you will find a collective expertise from various professional categories such as social workers, physiotherapists, dietitians, nursing assistants, nurses, medical secretaries, administrators, and doctors. Our goal is to prevent organ damage through prevention, improve patients' quality of life, promote engaged employees, and drive research and development. The role of a hospital social worker with us involves providing support and advice to patients with kidney disease and their relatives. As a social worker, you provide counseling support to empower patients and their loved ones to manage the stresses associated with the illness. Together with your colleague, you will be responsible for our transplant and donor reception, dialysis clinics, our nephrology outpatient clinic, and inpatient ward, as well as liaising with pediatric nephrology regarding the transition of young patients to adult care. You will also collaborate with other social workers in nephrology. As a social worker, you will also work on practical social measures aimed at supporting patients in maintaining a sustainable social situation and good adherence to their treatment. Living with a chronic kidney disease affects all areas of life, including family, relationships, work life, and future plans. Psychosocial support from healthcare is offered, and here you, as a social worker, play a central role. Living with a chronic illness requires access to the entire care chain. QUALIFICATIONS We now welcome applicants with a degree in social work and strong language skills in Swedish, both spoken and written. It is advantageous if you are a licensed healthcare social worker and have experience working with chronically ill patients. We are looking for someone with an interest in development projects and working with patients with chronic illnesses, which often involves long-term care relationships. Since you will collaborate closely with your social work colleague and other professional categories, teamwork skills, problem-solving abilities, and a strong interest in people are highly prioritized. It is essential that you feel secure in your role as a social worker, as there will be independent work involved. Personal suitability will be given significant weight.
