Make a Difference. Every Day. Are you an experienced social worker with drive and a desire to develop future care for children and youth with complex psychiatric difficulties? Then you might be the one we're looking for. In collaboration with Malmö City, Trelleborg, Svedala, and Vellinge municipalities, Child and Adolescent Psychiatry in Skåne has started a project, the first FACT (Flexible Assertive Community Treatment) team in Skåne. We operate from premises in the Kulladal area of Malmö and are staffed by an internal team of personnel from Region Skåne (doctors, nurses, psychologists, counselors, family therapists, nurses, occupational therapists, and unit manager) as well as personnel from Social Services ASF (family therapists). In our external team, we collaborate with coordinators from the other partner municipalities, schools, and other important care providers and stakeholders relevant to the cases. The clinic's target group is children and youth aged 10 to 18 and their families, where there is a complex child psychiatric issue requiring extensive support and treatment from both healthcare and social services. The purpose of FACT is to create a comprehensive structure for a treatment team around the patient where all relevant professions and resources deemed necessary for the patient's current needs are in focus and included. Flexible care on the patient's own terms is performed in close cooperation between healthcare and municipality. The meeting can be at the clinic, in the youth's home, or wherever they feel comfortable. Our goal is to guarantee continuity of care, raise functional levels, prevent inpatient care, and break youth social exclusion by stimulating active participation. The team should flexibly be able to meet needs of varying support intensity and interventions. The advantages for the youth are receiving adequate treatment and needs-oriented interventions from the same organization regardless of changes in their condition/state. Difficulties we often encounter with our youth include neuropsychiatric diagnoses, mood disorders, post-traumatic difficulties, and compulsions/OCD. Not infrequently in a complex problem with comorbidity and problematic school absence. The project's intention is to pave the way for more Child and Adolescent Psychiatry (CAP) units in Skåne to be further developed according to the FACT model. Working with youth and their families who have experienced long-term challenges requires a therapeutic approach, flexibility, and good collaboration skills. The assignment includes mapping, assessment, treatment, and investigation of youth with complex child psychiatric difficulties, often accompanied by comorbidity and social challenges. The complexity of our youth's difficulties requires skill in assessing and ruling out various symptoms and diagnoses, which necessitates interest, experience, and knowledge about the work required with our target group. We work closely with the youth and their social network, neighboring care providers, and other collaboration partners to achieve clarity on the interventions needed to improve the youth's condition and functional level. The work tasks involve both systemic and theoretical elements. Conversations can take place at the clinic, through home visits, or wherever the youth wants to meet. Mapping, observations, exposure, and activation. Individual and family conversations and collaboration are a significant part of the work with, for example, neighboring care providers, schools, and social services. As a team member, you will contribute beyond your specialist competence to the team's support of our youth when they need intensified interventions. As a counselor, you will have great opportunities to develop the counselor role with your colleagues and be part of implementing the overall work with SVP and evidence-based methods for stepped care, as well as further developing the FACT concept within CAP Skåne. Together with the work group, you will have the opportunity for monthly supervision. There are great opportunities for further education and training, and the clinic has regular lectures within the child psychiatric knowledge area. QUALIFICATIONS We are looking for someone with a social work degree. If you are a licensed healthcare counselor, this is highly meritorious. If you belong to a licensed professional group, language skills in Swedish corresponding to at least level C1 according to the Council of Europe's scale is a requirement. For non-licensed professions, good Swedish skills in speech and writing are required. Experience in psychiatry, preferably child psychiatry, and family therapeutic work is mandatory.
