Within Aleris' Palliative Care Area North, we provide care for seriously ill patients at home (ASIH). Care is delivered through healthcare choice on behalf of Region Stockholm. We offer specialized care tailored to each patient's needs, with the goal of providing qualified medical and psychosocial care around the clock.
In our operations, we care for adult patients with various diagnoses, most commonly cancer, COPD, heart failure, and neurological diseases. We take a holistic view of the individual, focusing on creating round-the-clock security for patients and their families through high accessibility, involvement, and continuity. Working in palliative care and ASIH means working preventively, providing individual care, and helping to create a sense of security.
The Role

As a social worker, you will provide qualified social support to help patients and their families manage and process their emotional, social, and practical situations arising from illness, injury, and changing life circumstances. The social worker offers expert guidance on issues such as illness-related crises, family conflicts, and social challenges. There is also a strong emphasis on interventions that create meaning and enhance quality of life.
We work in patients' homes but also have the option of receiving visits at our unit and conducting digital consultations.
You will be part of our paramedical unit with colleagues working in various professions including physiotherapists, occupational therapists, social workers, and dietitians. You'll have two social worker colleagues within Aleris ASIH North whom you can consult when needed. Beyond the paramedical group, you'll work closely with nurses, doctors, and nursing assistants. You'll participate in weekly team rounds together with all professionals.

We Seek Candidates Who:
• Have a social work degree
• Preferably have experience working with multi-morbid patients
• Are empathetic and can work with patients in complex care situations
• Are flexible, independent, enjoy teamwork, and confident in making independent assessments
• Have good communication skills and a solution-oriented approach
• Hold a valid B driver's license (requirement) as work involves meeting patients and families in their homes
Employment Terms
• Contract: Temporary until 2025-03-31, potential extension
• Scope: 80%
• Location: ASIH North, currently across two teams in Sollentuna and Upplands Väsby
• Start Date: As soon as possible
• Salary: Individual determination, collective agreement applies
