We provide health-promoting and treatment interventions based on each individual's unique needs and conditions, and have specialized knowledge about how disabilities can impact daily life, health, and development. Description Adult Habilitation provides health-promoting interventions for adults diagnosed with autism, intellectual disabilities (ID), movement impairments, or acquired brain injuries. We focus on each individual's unique needs and conditions and have specialized knowledge about how disabilities can affect daily life, health, and development. Habilitation works in interprofessional teams with medical and psychosocial expertise. We are now seeking a social worker for our facility located in central Gothenburg. We welcome your application! Job Duties As a social worker with us, you are an important team member. Work methods are varied and can be both investigative and therapeutic. Interventions are primarily provided to patients but may also include consultative support for key individuals in the patient's immediate environment. Collaboration and networking with other healthcare providers relevant to the patient's daily life are also part of the job duties. You will work using scientifically proven methods and clinical experience, with interventions provided individually or in groups during defined treatment periods. Work is primarily conducted in the facility's premises but can be provided in the patient's environment if needed. The work is diverse. You will be offered introduction, collegial supervision, opportunity for professional mentoring, and professional development work with other social workers within Habilitation in the region. Qualifications We are seeking candidates who have: Social work degree Knowledge of Habilitation's target groups Familiarity with Habilitation's interventions Experience with Asynja Visph (AV) Preferred are those who have worked in interprofessional teams. As a person, you must be able to critically examine your work in dialogue with colleagues and management. You also need a clear patient focus that you apply in collaboration with patients when selecting assessment and treatment methods. A significant part of the work involves collaboration with the team, colleagues, and other organizations and agencies. Additional Information Gothenburg Habilitation - Adult is a specialized facility with broad and deep expertise for our target groups. We provide interventions for individuals over 18 with congenital or early acquired permanent disabilities that cause difficulties in daily life. Interprofessional teams are composed of various professionals such as social workers, psychologists, occupational therapists, physiotherapists, speech therapists, nurses, physicians, and dietitians. Habilitation closely collaborates with various primary care, regional, and county healthcare services. Patient-centered care focused on creating value for the patient is crucial to us. To achieve this, we need to provide knowledge-based care while also being willing to explore new work methods. You will experience an exciting mix of tasks with opportunities to help shape the facility and develop new approaches based on the target group.
