Division of Social Care has the ultimate responsibility for children, youth, adults, and elderly persons who need support. The entire elderly care in Nyköping Municipality is part of the same operational area.
The Authority Office for Elderly Persons, which is part of the Social Care Division, consists of over 25 employees working as care managers, housing adaptation administrators, and administrative staff. WORK TASKS
We are now seeking a care manager who will work based on the Social Services Act (SoL). The care manager assignment involves ensuring equal and legally secure case management. Tasks include receiving notifications, applications, following up on existing interventions, documentation, investigation, decision-making, and guiding individuals based on their needs. You will meet people in need of help and support, conduct home visits, and participate in various collaboration meetings. In your work, you will collaborate and coordinate at individual, group, and organizational levels to ensure individual needs are met. You will have extensive contact with individuals, relatives/representatives, and other actors within the operational area.
At the workplace, we work in teams and have weekly team meetings for coordination and work support. Support in official duties is provided by colleagues, group leaders, and managers. If needed, there is also the possibility of receiving support from the municipality's legal counsel.
QUALIFICATIONS
Candidates should have a degree in social work or another equivalent higher education that the employer deems suitable. You should have good knowledge of current legislation and documentation. Previous work experience in official duties is meritorious. You have good computer skills and are articulate both verbally and in writing. You take responsibility for your tasks and have the ability to plan and prioritize work efficiently. We prefer candidates who are self-confident and have a professional and positive approach. As a person, you have a good human perspective and an interest in social issues. A driver's license is required.
