Case Worker in Disability and Social Psychiatry Services

Work Tasks As a case manager, you will investigate, make decisions, and follow up in accordance with the Social Services Act (SoL) and the Act Concerning Support and Service for Persons with Certain Functional Impairments (LSS). Our target group is young and elderly individuals who need support based on current legislation. This may involve interventions such as housing support, personal assistance, short-term stays outside the home, or housing with special services. The work includes meetings with service users, relatives, guardians/trustees, and collaboration with healthcare, other authorities, and partner organizations. We work in the LifeCare management system and investigate based on IBIC (Individual's Needs in the Center). About the Workplace The Social Administration's main task is to offer individualized support and services within elderly care, disability services, children and family, adult services, and financial assistance. We are present for municipal residents throughout all stages of life. The Unit for Elderly and Disability Services consists of approximately 25 employees distributed across different areas. Case managers work within elderly and disability services, LSS/social psychiatry, and a housing coordinator. The unit is led by a unit manager and two group managers. Once you are introduced and independent in your work, there are opportunities to work from home approximately two days per week. What We're Looking For To succeed in the role, you have: • Social work degree or other academically equivalent education assessed by the employer • Basic computer skills • Good Swedish language skills, both verbal and written It is meritorious if you have: • Experience with official authority and investigation methods according to IBIC • Knowledge of the LifeCare management system • B driver's license Personal Characteristics We're Seeking The case manager role is varied and places high demands on your ability to plan, structure, and quickly adapt.
