Habilitation Reception, Eskilstuna About Us We are a specialist resource within Region Sörmland, based on specific knowledge of permanent disabilities and their consequences in daily life. Our mission involves assessing and providing habilitative interventions for children, adolescents, and adults with permanent disabilities such as mobility impairments, multiple disabilities, traumatic brain injuries, intellectual disabilities, and autism spectrum disorders. The habilitation reception in Eskilstuna is newly renovated and beautifully located at Odlarvägen 3, next to Mälarsjukhuset. We operate from a unified reception in interdisciplinary teams divided into child, youth, and adult teams, with the patient at the center. We offer you a fun, stimulating, and developing job both individually and in a team. This position is permanent at the reception, starting in the adult team. Together with the rest of the team, you will work to provide the patient with the best conditions to live an active and independent life. The work requires you to be flexible and creative. We provide interventions both individually and in groups, either at our reception or in the individual's everyday environments. Much of the work is carried out together with the patient's network. We operate from a behavioral medicine perspective based on applied behavior analysis. We meet regularly at cross-county professional meetings to discuss and collaborate on profession-specific issues. Your Responsibilities Your role includes providing advice and counseling support to the patient, relatives, and others in the patient's network, as well as providing information about other community services. You will work with conversations (individually or in groups), education, coordination, information, and guidance. The role also involves close collaboration with other relevant entities in the community and within the region, such as the Social Insurance Agency and psychiatry. Your Qualifications For this position, it is a requirement that you are a licensed health and medical care social worker.
