About this role

Welcome to a well-functioning healthcare center! Salem Healthcare Center, with approximately 13,800 registered patients, is a well-functioning and stable unit known for its pleasant work environment. Our facility is located in beautiful premises in a scenic and quiet area characterized by villas, terraced houses, and a small number of apartment buildings. As Salem is a geographically small municipality, we have short distances between us and our patients. It is a friendly and enjoyable workplace where we care for one another and work towards the same goals. We prioritize a good working environment and high-quality care. The unit has an independent and experienced nursing team that runs its own clinics for diabetes, hypertension, asthma/COPD, and dementia. We have a highly competent psychosocial team with two social workers and a rehabilitation coordinator. We also provide primary care rehabilitation with physiotherapists, occupational therapists, chiropractors, and dietitians. Our mission also includes child health services (BVC) and family centers in collaboration with the municipality. The midwifery clinic is located in the same building. You can easily reach us from southern Stockholm and Södertälje, conveniently located near the E4 and commuter train. Job Responsibilities As a district nurse or nurse with further education, you will be responsible for your own clinic within your area. The position includes all tasks that occur in a nursing clinic. The work involves significant freedom under responsibility, where you plan your days and schedule visits in consultation with colleagues and the rest of the team. You will also work with chat and TeleQ, as well as provide home healthcare as needed. There is an opportunity for remote work one day every other week or as agreed with your immediate supervisor. Qualifications - District nurse or licensed nurse or with further education in diabetes or hypertension. - B driving license. - Proficiency in Swedish equivalent to C1. - Experience from previous work as a nurse and confidence in your skills. - Experience in primary care and the TakeCare journal system is advantageous.
