Health Center Wisby Söder
Here, approximately 45 employees work in interdisciplinary general medicine teams to provide our roughly 14,000 registered patients with good, close, and accessible care.

**Your Responsibilities**
With us, you will work both independently and in a team around the patient. The work is varied, broad, and educational, where we strive to utilize your and our shared knowledge, experiences, and areas of interest based on the patients' needs.
You will work within a broad generalist area and apply your knowledge to meet and care for patients of all ages, with a wide range of care needs, medical conditions, and functional abilities. You will closely collaborate with your nursing colleagues and other professionals at the clinic, where you will handle both planned and urgent visits, coordinate care, and adapt when re-prioritization is necessary. Within the group, there are specialist competencies in lifestyle habits, diabetes, heart failure, asthma/COPD, osteoporosis, and incontinence, where we strive to utilize competencies as effectively as possible in patient-centered work.
Your tasks will largely consist of telephone consultations and clinic work. You will make assessments, triage, and perform investigative, treatment, nursing, and medical tasks. You will be responsible for assessing care needs, triaging, and providing advice via telephone through TeleQ, as well as managing digital care cases via the 1177 e-service.
Clinic work includes tasks such as vaccinations, medication administration, examinations, and wound dressings. You will coordinate, plan, and execute nursing and medical measures in accordance with the patient's care plan. In some cases, you will act as a primary care contact for patients needing coordination, where you will play an important role in the team surrounding the patient based on individual needs and care plans.
**Who Are You?**
You are a district nurse or a licensed nurse. We prefer that you have experience in telephone consultations and coordinating patients' care needs.
You want to work with us for high-quality care, and you find it easy to create good interactions with both patients and colleagues.
We place great emphasis on your personal qualities and expect you to take responsibility and contribute with engagement and joy in your work. As a person, you are committed, solution-focused, and responsible.
You communicate clearly both verbally and in writing and have good knowledge of the Swedish language at a level corresponding to C1 according to the Common European Framework of Reference for Languages (CEFR).
Before a hiring decision is made, a background check will be conducted through an extract from the criminal record.
