Would you like to help run and develop our diabetes clinic together with a diabetes nursing colleague and collaborate closely with our doctors to care for our diabetes patients? Then this position is for you! The Centrum Health Center is a unit with approximately 14,000 registered patients, making us one of the largest health centers in Skåne. Here, we have a wide range of expertise within our outpatient services, including district nurses, specialist doctors, resident doctors, nurses, nursing assistants, social workers, occupational and physiotherapists, and dietitians. It is important for us to offer our patients good accessibility, respectful treatment, continuity, security, and high medical competence. We have a strong interest in mentoring and often host students from all professional categories. We are currently in a development phase where we are actively working to improve our outpatient services and further strengthen teamwork within the organization. JOB RESPONSIBILITIES As a diabetes nurse with us, you will encounter various types of diabetes patients and work with, for example, adult patients with diabetes, newly diagnosed diabetics, and patients with prediabetes. You will conduct planned follow-ups of our diabetes patients and make registrations in the National Diabetes Register (NDR). You will be engaged in the development of our diabetes clinic with high medical quality and professionalism. You are expected to stay updated on diabetes knowledge and maintain close collaboration with doctors, dietitians, and others at the health center to offer patients evidence-based diabetes care. To support your professional development, we offer regular and relevant continuing education. In addition to your own outpatient work with diabetes patients, other duties at the health center will include telephone counseling and triage. QUALIFICATIONS We welcome you who are a licensed nurse, or alternatively a district nurse, with the authority to practice within Swedish healthcare. Additionally, you have further education as a diabetes nurse. Proficiency in Swedish at least at level C1 according to the Council of Europe’s scale is a requirement. Furthermore, it is advantageous if you have work experience from primary care and/or experience working with the PMO medical record system. The person we are looking for is confident and flexible in their professional role and comfortable working both independently and collaboratively with colleagues from various professions.
