We have approximately 4,700 listed patients and around 450 patients with type 2 diabetes. We consist of various professionals, we are a cheerful team working in pleasant facilities. Working in a sparsely populated area can be challenging, but we are very solution-focused and solve most things, working in a person-centered manner.
We are now seeking a Diabetes Nurse for our team! As a diabetes nurse, you are the conductor who coordinates diabetes care at the health center. You are responsible for planning, follow-up, and treatment of diabetes patients together with physicians.
You will be offered regular internal and external skills development as needed. The introduction will be adapted to your specific requirements.
The position is a permanent full-time employment with immediate start or by agreement.

Job Responsibilities
• Manage and plan diabetes care
• Follow up and participate in treatment of our diabetes patients
• Have a coordinating role for diabetes care and drive development and quality assurance of diabetes care at the health center
• Supervise students and other staff regarding diabetes care at the clinic
Qualifications
We are looking for someone who:
• Is a district nurse with diabetes training
• Is a nurse with experience working with diabetes patients
• Has excellent Swedish language skills (spoken and written)
• Holds a B driver's license
Preferred qualifications:
• Experience in diabetes care
• Work experience in primary care
