Welcome to Us The Adult Unit works with individuals struggling with dependency issues and comorbidity and currently consists of 19 employees: 9 social workers, 4 case managers, 1 housing guide, 1 eviction prevention specialist, and 1 housing coordinator. The unit is led by a unit manager and 2 group leaders. We work with both administrative tasks and supportive interventions. The unit is part of the adult area together with financial assistance and RVC. Collaboration is crucial to achieving our goals, and we continuously work to improve it. We are now seeking a case manager with commitment and interest in working with individuals with comorbidity. Your Role Case Management is an intervention with a coordinating function aimed at enabling individuals with comorbidity to live as independently as possible. Many users you'll work with have interventions like training or trial apartments. Work Tasks: - Outreach social work for users with comorbidity - Implementing assignments developed with social workers and users - Identifying and mapping user needs for rehabilitation process progression - Supporting contact with social services and other collaborative partners like psychiatry and addiction care Case management involves significant work outside the office. You'll work outreach, often meeting users in their home environments or at their living spaces. Evening work may occur. Your Competence and Experience To be considered, you need: - Social work degree or equivalent education - Motivational Interviewing (MI) training Meriting factors: - Case management education - Case management work experience - Administrative and service delivery experience - Knowledge of addiction, psychiatry, neuropsychiatric diagnoses, and cognitive disabilities - Strong Swedish communication skills (verbal and written) We seek someone who independently drives work forward. You have perseverance and continue working through challenges. Clear, pedagogical communication with professionals and users is crucial. You have a solution-oriented approach, ensuring information reaches both users and professionals. You can synthesize information and independently communicate with relevant parties. Personal suitability is highly important. To work with Stockholm City's children, youth, elderly, and persons with disabilities (including home-based work), you must present a criminal record extract no older than six months.
