About this role
At the Authority for Care and Welfare in Vellinge Municipality, there is a first officer, eight assistance officers under the Social Services Act (SoL), three LSS officers, one fee officer/housing coordinator, one fee officer/elderly and relative guide, and two senior contacts.
The position advertised is a permanent role as an assistance officer in our interdisciplinary collaboration team for discharge (SVU), which consists of two assistance officers, a nurse, and an occupational therapist. The collaboration at discharge (SVU) is a legally mandated process aimed at ensuring a safe and smooth transition for patients from inpatient healthcare to home or municipal care.
JOB DESCRIPTION
As an assistance officer, you will be responsible for investigating, making decisions, and following up on interventions according to the Social Services Act and the National Travel Service Act. As an officer, you are responsible for collaboration and information throughout the ongoing investigation. The work is based on a holistic perspective surrounding the individual, with their needs at the center, which occurs in cooperation with your immediate team as well as with relatives, healthcare services, other authorities, and care providers.
The profession of assistance officer involves interactions with various individuals, thus requiring social skills, being receptive, and an interest in creating meaningful meetings. You enjoy meeting people, have a positive outlook, and possess strong collaborative skills coupled with a personal drive.
To succeed in this role, you need the ability to create clarity and trust. In meetings or dialogues with individuals and their relatives, a respectful approach and a warm interest in working with people are very important.
The position further requires independence, flexibility, a sense of responsibility, and the ability to communicate clearly and trustfully, both orally and in writing. In addition to formal qualifications, significant emphasis will be placed on personal suitability.
QUALIFICATIONS
Education:
We are looking for candidates who have a degree in social work, a degree from a social care program, or other education that the employer deems equivalent.
Knowledge:
Good knowledge of current legislation and legal practice in the area, as well as experience in exercising authority under the SoL and the SVU process.
Experience in the IBIC (Individual Based Integrated Care) model is desirable.
Strong IT skills. Experience with the operational systems Lifecare and Mina Planer is an advantage.
A valid driver’s license (B category) is required.
