Your Workplace The mission of the Rehabilitation Unit encompasses rehabilitation for patients within municipal primary care and is part of the Health and Support Services area. The unit shares an office with the nursing unit at Hönekulla Gård, which provides care, support, and short-term housing in Mölnlycke, and is responsible for all geographical areas in Härryda municipality. The unit employs occupational therapists and physiotherapists, totaling 19 staff members along with a unit manager. Improvement work is ongoing within the organization to enhance quality and patient safety in care-related teamwork. This is achieved by strengthening licensed professionals in their roles as quality leaders based on their professional responsibilities, as well as developing structured collaboration forms within the unit leadership team (ELT) in care and support, and creating collaborative frameworks with functional support to enhance quality assurance. Your Mission Are you a licensed occupational therapist interested in a developing and meaningful job where you can make a difference for individuals receiving support from municipal primary care? We are seeking a new colleague for our rehab team as one of our team members is relocating! - As an occupational therapist, you will work from a person-centered approach focusing on the patient’s resources and participation. You will assess the patient’s ability to perform activities based on physical and cognitive functions as well as the surrounding environment. - You will plan and implement interventions based on the patient’s goals, assess the need for assistive devices, prescribe assistive devices, and write certificates for applications for housing adaptation grants aimed at maximizing the patient’s activity and participation in daily life. - Interventions are carried out in the patient’s home, i.e., in ordinary housing, care and support housing, housing with special services, daily activities, and short-term units. - You will instruct, guide, and educate patients, relatives, and care and support staff in rehabilitation efforts. - The work is based on a team-oriented approach in collaboration with physiotherapists, nurses, rehabilitation representatives, staff within care and support, and social workers. Requirements for the Position - You are a licensed occupational therapist with an interest in team collaboration and developing safe and secure rehabilitation within municipal primary care. - You possess pedagogical skills to guide and instruct patients, staff, and relatives based on occupational therapy interventions. - You have knowledge of cognitive disabilities regarding assessment, support needs, strategies, and the prescription of assistive devices. - You are well-acquainted with regulations and guidelines for the prescription of assistive devices and have experience and confidence in assessing needs, trialing, and following up on personal prescriptions of assistive devices. - Experience within municipal primary care is an advantage. - A valid driver's license (B) is required. - Good ability to express oneself in Swedish, both orally and in writing, as well as good computer skills are required. To obtain employment, a criminal record extract from the police must be presented.
