About this role
Hofors Primary Health Care Center conducts conventional primary care with physicians, child healthcare center (BVC), district nurse and nursing reception, nursing assistant reception, sample taking, psychosocial team, occupational therapist, physiotherapists, rehabilitation coordinators, and medical secretaries. The premises also include a maternity healthcare center. We have approximately 40 employees at our unit and hold regular professional group meetings.

JOB RESPONSIBILITIES
We are seeking an occupational therapist who will work 60 percent as an occupational therapist and 40 percent as a rehabilitation coordinator. You will also participate in collaboration with other occupational therapists and rehabilitation coordinators within our Primary Care Western Gästrikland area.
As an occupational therapist, you will primarily work on:
• Assessing activity capacity in consultation with patients and rehabilitation needs
• Assessments, aid prescription, fitting, follow-up according to the prescription process
• Memory assessments to complement district nurse and physician evaluations
• Formulating statements for housing adaptation and personal assistant applications
• Controlling aid costs and rental responsibilities
• Patient education and counseling
• Individual treatment, largely in collaboration with other professionals
• Assessments and occupational therapy interventions in patients' homes
As a rehabilitation coordinator, you will collaborate with and support physicians, other professional categories, patients, and unit managers in sick leave work. You will also be the unit's contact person for involved parties in the sick leave process.
Rehabilitation coordinator work involves support for sick-listed patients needing assistance with work-oriented rehabilitation. The assignment is based on the coordination law that took effect on February 1, 2020, and encompasses three areas:
• Personal support for sick-listed patients needing assistance returning to or entering the workforce
• Internal coordination within and between units for consistent approach to individual patient sick leave and rehabilitation processes
• Collaboration with other actors by contributing healthcare perspectives to facilitate interventions within their responsibility areas
• Coordinator for the unit's Trisam team, structural collaboration between Employment Agency, Social Insurance Agency, municipality, and healthcare

Qualifications:
The role requires ability to work independently and take initiative in developing the position. Great emphasis is placed on personal suitability.
As an occupational therapist, you should:
• Be a licensed occupational therapist
• Have basic knowledge of medical terminology and experience reading and documenting in medical records
• Hold a valid Class B driver's license
