Home Rehabilitation Means Great Freedom.
For those who work and for those who cannot visit the rehabilitation clinic. At Bräcke diakoni, you will work independently, close to people's everyday lives, and make a real difference every day. We are now seeking an occupational therapist who wants to cover for a colleague on parental leave.
Start date can be adjusted according to your preferences, but we would prefer you to start in December. The position covers between 75 and 100%, and we are happy to find a solution based on your possibilities and the organization's needs.
Our Organization
At Bräcke diakoni, we are proud to take comprehensive responsibility for care, healthcare, and rehabilitation at home in Linköping. Our home healthcare team collaborates with the municipality and several healthcare providers to ensure those in need receive the right help. Many are elderly and individuals with disabilities who want to continue living at home. Therefore, we actively work to manage health problems and strengthen existing resources. The team consists of nurses, physiotherapists, and occupational therapists who collaborate and create holistic care. The team also includes nursing assistants from Bräcke's home care groups working in homes and sheltered housing connected to home healthcare.
Your Role
During your employment, you will be part of the rehabilitation team, traveling during the day to meet patients at home, in Bräcke's sheltered housing, and home care units in City South. You will be based at Stolplyckan, sharing an office with your colleagues.
Job Responsibilities
- Functional assessments
- Planning and performing rehabilitation interventions with patients
- Adapting living environments and selecting technical aids
- Supervising care personnel
Who We Seek
We are looking for a licensed occupational therapist who can plan safe and health-promoting rehabilitation interventions in home environments. This involves being attentive to individual needs and actively collaborating with patients, colleagues, and contacts in the patient's network. With previous professional experience, you should be prepared to supervise care personnel and participate in prioritizing interventions.
Qualifications
- Licensed occupational therapist
- Valid B driver's license and ability to cycle
- Fluent in Swedish (writing and speaking)
Preferred
- Experience in home rehabilitation, special housing, or disability services
- Experience working with the Treserva journal system
